To celebrate the first ever National Social Prescribing Link Worker Day (8th July 2019), we caught up with Angi Hartley, Practice Manager at Mill Road Surgery, to find out how they use social prescribing and how it benefits clients.

“In General Practice we see patients who may access our services for medical related problems, but sometimes they may have other factors which are impacting on their lives and contributing to their ill-health, such as isolation, problems with housing or financial worries etc. Having access to a Social Prescribing Link Worker enables us to treat the patient holistically – the Link Worker is able to assess all the patients needs and liaise with the most appropriate teams to address all the patients’ concerns. Outcomes are fed back to the GP for information and patient feedback from the service has been positive.”
